Parent Survey Part B (ages 3-21)

Parents, to get an accurate picture of how Wyoming is doing please complete the following survey. Also, if you know anyone else that this survey applies to, please pass the information along to them. For your convenience we have provided multiple ways to complete this survey:

  1. Online survey, complete the quick 3-minute, 15 question survey found below (scroll down) or
  2. Printable version (.pdf) may be filled out on your computer or printed out then completed by hand and mailed to PIC or
  3. Call PIC at 307-684-2277 and complete the survey over the phone

The survey ends September 13, 2010, please complete before then!

What is PHP of Wyoming, Inc.?

Parents Helping Parents of Wyoming, Inc is a private, non-profit organization founded in 1990. Governed by a board of directors, the majority who are parents or family members of children with disabilities, PHP 's mission is to help families become more active in their children’s learning and education, to serve and support children with and without disabilities through advocacy, education, and referral. PHP of WY is a non-profit, 501(c)3 organization; therefore, your donation is tax deductible. We welcome all donations. Every donation helps us strengthen our network of support for families.
